Need an immunization form for college?

My Question: Need an immunization form for college?
I need to fill out an immunization form for college AND it needs to be signed by a medical professional. I have an old immunization record sheet with all the required immunizations but it’s not on that form and the school wont accept it! I called my health care provider (the one I usually go to) and they want $ 100 for the doctor to sign the document as a “form fee”!! Is that high? And is there anywhere I can get this done online via email or fax for cheaper?


We were able to send colleges my son’s immunizations forms from his high school. His High school had to send them direct to the colleges.


Try your local health department; ask if there is a nurse that can review and transcribe the immunization record.

By I Am Chaos: Submitting college application personally to admissions office?
I want to know if its possible to submit the application personally, by driving to the office of admissions at George Mason University and handing them my supporting documentation personally. George Mason has been known to misplace some supporting documentation, but my main concern is if I were to mail it, it might arrive past the application due date. I am applying today online, but they wish for you to send supporting documentation (such as transcripts, letters of recommendation, counselor reports etc.) my mail, e-mal, or fax. I don’t want to risk my documents arriving past Janurary 15th. Geroge Mason is about 20 minutes from here, so do you think they would allow me dropping it off personally. Has anyone ever dropped an application off personally? I’ll have everything I need by Monday or Tuesday, but will it arrive their by January 15th (next monday which is Martin Luther King’s Birthday) if the school is 20 minutes away? Please answer all questions it would be a great help to me!

Im sure theyll accept it just deliver it to the office of admissions
i sent my application online but i delivered my credentials (transcript, letters of recommendation) personally at UT. just make sure you get a receipt when you turn it in, and you should have it all in an envelope..good luck!

Wow…this is not necessarily the answer you want to hear, but you have to check in with the admissions office. My former university would accept admissions applications in person, but before the deadline. You don’t have to tell them “why” you want to submit it in person. Good luck.

If you are really concerned, then dropping off your application personally should alleviate your concerns. However, if you only live twenty minutes away, it will only take a day to have your application materials arrive by mail. I would only worry about mailing your materials if you lived in an international destination.

Every university I’ve dealt with will accept hand delivery. But I’d suggest you call ahead to make sure you get there during office hours.

Also–most universities have on-line options for a lot of this paperwork now–and that speeds things up considerably. You should ask what your options are in that regard as well. Also check on faxing some documents if that seems appropriate.

NEVER TRUST THe PO. I am still waiting on a priority mail package that was mailed in Aug. Def. hand deliver it and get a rec. for it.. As far as transcripts, they have to be the official transcripts MAILED from the schools.. U may be able to hand deliver the rest but not official transcipts..

“FREE” college- good scare tactic, but, it won’t work on anyone that understands the college process. Offering in-state tuition is not FREE tuition. It is the price that people who have lived in that state receive. The states that offer in-state tuition simply define “resident” as someone that resides in their state. So, the act is offering in-state tuition to:

People that reside in that state

WEre here before 16 (and for more than 3 years- so 13 or younger)

Are currently under 30 years old

Have to spend 2 years in college or military

Are granted 6 year resident cards, at the completion of the 6 years if the requirement are met, they can *apply* for citizenship. Or, if they haven’t they will be illegally here.

The way people try and rile one another up is simply ridiculous. READ THE BILL yourself. It isn’t really a boogeyman coming in the night to steal your childrens future. The failure of another student doesn’t assure the success of yours.

If you read the Dream Act, it’s also gives them a quick amnesty and a fast track to citizenship. I’m even MORE concerned about that.

My college needs a copy of my 2007 tax return to process my financial aid application. The harddrive that is holding that information took a dump. IRS will send a transcript copy but it takes 2 weeks. College said that they would accept a faxed copy (and that they have in the past), but they don’t know how to do it. IRS has everything automated and I cant request a fax. Anyone know what I’m supposed to do?

This makes a good case for always printing out a copy of your tax return for your file.

What is wrong with just duplicating the return? If you don’t have the software, someone else should have it. You still have your data from which you prepared the return, right?

You can ask IRS to fax the transcript to you and then you can fax it to the college. I doubt they’ll fax it directly to them but it won’t hurt to ask. If they won’t fax it at all, guess you’ll be watching for the postman.

The IRS will fax a transcript! I’ve done it before. Just provide the college’s fax number. Faxes usually are delivered within 10 to 15 minutes.

Go to your local IRS office and request the transcript it will be given to you while you wait.
Or contact IRS at 1-800-829-1040 and they can fax it to you but you have to be where there is a fax machine and you must state on the phone line with the CSR until you receive the fax.

The problem is that you have to be present at the fax machine when it is faxed, and the automated system the IRS uses will make multiple attempts within a 48 hour period–thus there is no guarantee you will be present when the fax is actually transmitted. Sometimes the representative will be willing to manually fax the information while you’re present at a fax machine, but that’s up to the policy at the particular call site you’ve reached.

The other option is to fill out Form 8821, granting the financial aid office the authority to receive your tax information. You will then have to call the IRS and be able to fax that form to the representative while on the phone.

Every place I call will not fax or email application. What do I do? I need to move from where I am to there. I cant afford to fly out just to fill out an application and I can’t risk moving and then doing my application. Plus Fall classes start soon and everything is filling up.

This sounds odd. Most properties love out-of-town renters so they can give them their least appealing rentals. :) Concentrate on larger, professional-managed properties or offer to send your application and deposit payment via FedEx.

If classes are starting soon and you’re applying in high-demand areas near your school, then landlords may just prefer students who are able to pay and apply immediately without having to wait for an out-of-towner.

Try having your parents apply. A landlord may be more cooperative if they believe a non-student is applying for the apartment. You can then be added as an additional occupant.

Also, consider shared housing situations. There may be students already in town who are seeking an additional roommate.

I’ve been accepted into a college(UCD) with my legal name. But right now, the name that’s on my high school transcript and a transcript of a summer local college class I took is not my legal name. It’s cause when I immigrated to the US from South Korea as a child, my name was separated- one syllable as my first name and the second syllable as my middle name- on my passport, so obviously, they were separated on my greencard because of my passport… But on my high school transcripts, the two syllables are joined together as my first name with no middle name. My driver’s license will be like that when I get it.
I’m 18 now and I’m going to apply for citizenship soon, and join the two syllables as my first name.
Should I change the name on my college application or my high school transcript? Is it ok if they don’t match up. I’m assuming they have to be?. I checked UCD’s name changing process and it says to fax my SS card. What if the name on my SS card is not the name on my app?

It sounds like you already are a little confusing with records because the high school records are different than your passport and green card. So no matter what you do, there is room for questioning, but since is it a fairly minor difference because of a space break, I wouldn’t worry too much.

Your high school records show your name the way you want it to be. I would fill out college applications to match that name, but in a letter I would include a BRIEF explanation that you are in the process of legally changing your passport and other documents to match that name.

If you have already started the application process with a school and used the other spelling, I wouldn’t change in the middle of the application process but would wait until accepted and then change it after you enroll because making the change in the middle of the process would be more confusing than waiting.

I am speaking from personal experience because I was in the EXACT same situation as you were when I started college. My advice is, when you apply for citizenship, pick the name that you like the best because you will be stuck with it for the rest of your life. Changing your name after obtaining your citizenship is very difficult. You have to go to a judge and it’s a complicated process.

I may even consider changing it to an American name, unless your Korean name sounds OK in English. Your name is very important since people make presumptions about you if they only see your name and don’t know you personally. Sometimes people assume that someone with a foreign name doesn’t speak English or may be difficult to get along with because of the cultural differences.

Your school wanted to see a SS but they should let you substitute it for something else, especially if you explain them your situation. Usually when you submit an application, there is a space for former names used. Besides becoming a citizen, some people get married in college and change their last names so school should be flexible enough to deal with name changes. My point is that pick the name you want instead of trying to “please” the college. After all, your name will be there with you beyond college.

There shouldn’t be a problem. It’s the same name except joined together. Everyone would understand. As long as there no fraud involved the U.S. allows people to change their name and minor spelling changes are always allowed.

So I just graduated 2 months ago with a B.A. in Journalism, but I soon realized freelance writing was a very unreliable salary!

I put my resume online and was called by this Software Consulting company that makes software for medical billing offices and insurance offices. The job description listed $ 30,000-$ 45,000.

They had me in for an interview, and by the time it was over, they had handed me a packet with info about the job, drug test papers, etc plus a job salary offer of $ 36,500. This includes 401k (with price matching), full health care coverage not taken out of the salary, 3 weeks paid vacation and a 15% end of year bonus.

I realize I don’t have application consulting experience but they were impressed by my resume and educational background. I think I could pick it up pretty easily. They told me there are opportunities for advancement within the company. It is only 7 years old and expanding.

Anyway, the interview was Monday and they said to have the papers signed and faxed to them by end of business day Wednesday (today!)…

This is my first job and I don’t want to push my luck. Should I counter-offer or should I be happy with what I was offered? Most people have told me they think this is a pretty good deal for an entry level job, but a few have said they were surprised I didn’t counter-offer. And if I were to counter, how would I go about doing so? Call, email, fax? What would I say? How much should I ask?

I really don’t want to make things awkward before I even start the job! I am nervous they won’t go for it and then I’ll have made things harder on myself… Help please :)

You’ve been offered a ,500 and your not entirely happy? SOrry but when a job descrip says between ,000 and £45,000 you are likely to get the minimum. You’ve done well to be offered what you have. Its fantastic for an entry level job role. And theres possibilities of increasing that wage through career advancement! Also in entry level jobs, there will be enough people happy to take that sort of wage that if you counter, i expect the job will go to someone else. Getting a job is so difficult for some people at the moment that you should seize the opportunities given to you.

you got a better than average offer – no one ets 3 weeks vacation their 1st yr – I had to wait until I was at my current company 5 yrs and you starting wage is right in the middle – in this economy, with no relevant job experience, you do not have any negotiating power – be happy you got an offer. there are plenty of other people who will say yes if you don’t
