by the phone company? For instance, would faxing to a long distance number be treated like a long distance call, a toll-free number would be toll free and so on?

No, faxing usually has one set price per fax but beware, it can be expensive in the long run. Our monthly bill was huge because of us having to use our fax machine a lot (we’re in the uk so pricing varies country to country)

Yes. as far as the phone company is concerned, you are making a station to station call, and all tolls and tariffs apply as if it were a voice call, and there are no extra toll or tariffs.
The fact that either end are machine and the communication is not voice is not relevant.
